The SIM card will only fit the adapter one way. The SIM card adapter will only go into the router and lock (click ) into place one way.

The router is seeing the modem. So that is a good thing.
Assuming your r.ispsn is the correct APN for your device, I would check to make sure it is reading the SIM.

You may also want to try programming and locking the bands. I have heard others claim of having to program and lock on band 41 to get carrier aggregation. Let me know if you need programming instructions.

Just curious, have you modified any other settings in the router firmware trying to get a connection? The router appears to be detecting the modem so that is a good sign.
If there anyway, using the Huawei E397, you can check to assure that fast.t-mobile.com is the correct APN. Seems like I have seen others at time being used.Entered in APN info for T-mobile (fast.t-mobile.com)

This may seem like a dumb question, but is your antenna pointed towards the cell tower, or are you using the SMA screw on LTE antennas?

It appears to be connecting and immediately disconnecting. First, make sure the correct APN is entered in MODEM >> CONNECTION INFO >> APN and there are no blank spaces behind the APN. Next, go to MODEM >> DEBUG INFORMATION >> AT COMMANDS and copy and paste what it is showing.

Most are using the Sierra Wireless MC7455 for custom builds. They will accept a lot of configurations to include locking bands.
A lot of builders use the MC7455 with our WE|WG line of LTE routers. There is a tutorial for building the WG3526-P.

It does sound the SIM card is bad since the hotspot cannot read the phone number. They can be finicky at times. You may try removing the SIM card and cleaning its surface with an alcohol pad.
