You have to set the IMEI, then set the carrier profile to generic, then reboot, then set the APN to fast.t-mobile.com, then reboot.

I would connect to the uart/serial pins over usb and see what the serial terminal shows is going on. It might have a failsafe tftpd server/client flash recovery.

The newer builds require you to delete the IPV6 prefix on the Network->Interfaces global options/settings tab at the top of the page. Delete the setting there, leave blank, and reboot.

You've done a thorough troubleshooting job.
I'd say something simply failed in your LAN driver chip onboard. The only thing that came to mind was the cables and connection points, but you covered that too. Looks like next step is the pcb itself.

I think it should work fine as long as you are not jumping any antenna ports together, IE using both modem ports on the one antenna in parallel.
Using just the main ports on two polarized elements shouldn't create any significant interference, unless it has some odd design I haven't considered.

Nope, it's not running any routing jobs.

Nope. The modem is a separate OS from the router. OpenWRT based firmware doesn't run modem reset config scripts when the router firmware is reset.

Set your APN to ipv4 only.. it's a setting under modem->connection profile. If needed, disable the modems ability to auto load apn's via at command.
For local, that's under network->interfaces->lan

Mesh+ doesn't have recovery that I can figure out. They use a custom u-boot that seems to not have it.

PCB antennas are omni-directional antennas; typically dipole.
Directional antennas generally would require an outdoor installation.

Generally they will all reference the same grounding point. I don't think it is an issue for these type of routers and modems. I could be wrong, but that is my thoughts on it. There is some debate on this. The only way to tell is to have the ping6 utility/app on a pc and ping the routers ipv6 address, if there is one available. regular ping for ipv4.

The image is mislabeled primary-secondary.
I would connect 2x2 mimo to ANT0 and ANT3. Those two cover all bands. ANT1 and ANT2 do not.

As long as the cable is outdoor rated, it'll be fine. You can use up to 300 feet for 48V PoE. I don't really recommend over 100-150 feet.
