I believe these were my problems along the way: I had initially followed the instructions correct; however, I did not have the extra custom TTL Rules in there. When I did put them in there, I replaced what was already in there instead of adding to it, so I was still down. Then, in trying to diagnose...

Hmmm... found a thread about making the modem 'GENERIC' rather than visible. Re-ran that. Switched back to port 64 and seem to be up and running again.
